There are five key handoffs that shape the patient’s experience:
- Front Desk → Clinical Team (Assistant or Hygienist): Set expectations for the visit, relay patient concerns or updates.
- Assistant or Hygienist → Doctor: Share patient goals, notes, and emotional context - out loud, in front of the patient.
- Doctor → Clinical Team & Patient: Confirm what was done, what’s next, and ensure everyone is aligned.
- Clinical Team → Front Desk: Communicate next steps for scheduling, financials, or follow-up.
- Front Desk → Patient: Confirm the plan, schedule visits, and reinforce the next steps.
When these moments are intentional and consistent, patients feel safe, valued, and confident in the plan.

How to Master the Patient Transfer Process
1. Define the Key Transitions
Focus your team on these high-impact moments:
- From front desk to clinical team: Set the tone.
- From hygienist to doctor: Transfer context in front of the patient.
- From doctor back to team: Confirm next steps.
- From clinical team to front desk: Ensure clarity for scheduling and financials.
2. Always Transfer in Front of the Patient
Let them hear you say the same plan more than once. This boosts comprehension and confidence.
3. Rehearse Daily. Review Weekly.
- Start with reminders at your morning huddle
- Role-play in weekly team meetings
- Review real cases where the handoff worked (or didn’t)
4. Monitor & Improve
- Perform daily walk-throughs and callouts
- Weekly highlight reel: What went well? What missed?
- Once the system sticks, move to monthly reviews
Common Mistakes to Avoid
- Incomplete Handoffs: If even one detail gets dropped, it erodes trust and can cause delays or confusion.
- Silent Transitions: When conversations happen behind closed doors, patients feel disconnected.
- Inconsistency: If only some team members follow the system, it will never stick.
