What’s my practice worth?

When should I sell?

What multiple will I get?

But this:

When I’m gone, what will my team say about how I led?

Most dentists spend decades building a practice. They invest in equipment, technology, marketing, clinical skill, and growth.

Very few invest intentionally in leadership legacy.

And yet, when the time comes to transition, leadership - not numbers - is what determines whether the experience feels like a graduation or a loss.

Because here’s the truth most transactional conversations never address:

Your sale price reflects what you built.

But your legacy reflects how you built it.

And the dentists who transition well - the ones who walk away with both financial security and personal peace - understood that distinction long before anyone talked about selling.

The Leadership Gap Most Dentists Don’t See

Over the years, I’ve observed dozens of transitions - both personally and alongside trusted partners.

Some were smooth. The seller walked away proud. The team stayed intact. Patients felt continuity. The culture held.

Others were painful.

Staff left within months. Patients felt unsettled. The culture eroded quickly. And the seller - despite getting their number - felt hollow.

The difference wasn’t the market.

It wasn’t timing.

It wasn’t even deal structure.

It was leadership.

The dentists who transitioned well had built something that could outlive them.

The ones who struggled had unknowingly built something entirely dependent on their presence.

That’s not a valuation issue.

Most dentists fall somewhere between survive and collapse.

They look at dependency risk, decision bottlenecks, team autonomy, patient retention when you’re gone.

I’ve seen one practice transition where systems were strong and the team barely missed a beat - and another, just as profitable on paper, where staff turnover and cultural erosion began within weeks because everything had lived in the owner’s head.

These aren’t exit tactics.

They are leadership disciplines.

And they determine whether a transition is calm or chaotic.

How to Start Building Leadership Legacy This Week

Leadership legacy sounds abstract. It isn’t.

Here are five concrete actions you can take this week to start building transferable leadership:

1. Document one critical process

Pick one thing only you know how to do. Write it down. Put it in a shared folder. Train someone else.

2. Delegate one decision permanently

Choose one recurring decision and give someone else full authority.

3. Ask your team the legacy question

“If I stepped away for three months, what would break first?”

Listen without defending.

4. Create one leadership layer

Give someone ownership of outcomes - not just tasks.

5. Test your systems

Take a real vacation. See what breaks. Fix it when you return.

If you can’t leave without the practice suffering, you don’t have a business. You have a job.

The best time to build leadership legacy was ten years ago.

The second-best time is this week.

Legacy Is What Survives You

Legacy isn’t your sale price.

Legacy is what remains standing when you step back.

Would your team feel protected?

Would patients feel continuity?

Would the culture hold?

Those answers aren’t decided in negotiations.

They’re decided by leadership habits repeated over years.

Why Leadership Must Come Before Representation

Representation matters - but representation cannot replace leadership.

If leadership hasn’t done its work:

  • Representation becomes damage control
  • Negotiation becomes defensive
  • Outcomes become compromised

But when leadership is in place:

  • Representation amplifies strength
  • Buyers are evaluated, not chased
  • Regret is minimized
  • Control is preserved

This is the difference between a transaction and a transition done well.
